Executive Physical Exams

The executive physical examination is a much more intensive evaluation of a person’s potential for disease.

Extensive time is spent with the doctor going over medical history, and discussing any issues that might otherwise not mention in a standard physical; even minor symptoms may be viewed as relevant and deserve scrutiny.

Personalized Care

Bayview General Medicine Concierge Program is not Medicare, an HMO, or a PPO. Bayview General Medicine offers a program that is intended to be separate from Medicare and insurance health plans. In exchange for the Participation Fee, Bayview General Medicine agrees to provide the following Amenities to its participants, none of which are believed or intended to be covered by any insurance or Medicare reimbursement:

• Same Day or Next Day Appointments for non-emergency matters
• Appointments with minimal or no wait time
• 24/7 access to a physician for non-emergency matters
• Special email portal access
• Periodic email updates to Participants on timely and pertinent health topics
• Appointments for out-of-town family members in need of unexpected care (during visits no longer than two (2) weeks) on a fee-for-service basis
• Access to Participant’s lounge for family members during office visits
• Assistance in handling medical needs while traveling

THIS IS NOT A CONTRACT FOR INSURANCE. Participant understands that this Agreement is a participation contract only, and not a contract for insurance or the provision of any medical services whatsoever. Participant acknowledges that he/she is not entitled to receive any medical services from Bayview General Medicine as a result of the Participant’s execution of the Agreement. The Participant acknowledges and agrees that the Participation fee as provided for herein does not constitute payment (in whole or in part) for any medical, clinical, diagnostic, or therapeutic services, does not constitute a payment or premium for any insurance benefit whatsoever, or payment for any items that are covered (in whole or in part) by any third party payor providing benefits to Participant. Participant represents and agrees that the Amenities and the Participation fee are not covered or reimbursable by any third party payor, nor covered by or paid for in any manner by any co-payment. Neither Participant, Bayview General Medicine nor any of their respective agents or employees will seek reimbursement or payment from any third party payor for the Amenities provided hereunder, or any portion of the Participation fee. The Participant also acknowledges that these Amenities as listed above, and any others provided under this Agreement, are not covered by insurance, are not reimbursable by Participant's insurer or other health plan, not part of any co-pay, and are subject to change or modification without notice.
