Immigration Medical Examinations

The following information is taken directly from the US Citizenship and Immigration Services (USCIS) Website:

Medical examinations are needed for adjustment of status cases (Form I-485) and requests for V nonimmigrant status (Form I-539). The medical examination must be conducted by a medical doctor who has been designated by U.S. Citizenship and Immigration Services (USCIS) as a civil surgeon.

You cannot go to you personal doctor to have a medical exam for immigration purposes unless your doctor is a designated civil surgeon. (Dr. Blanchar is a designated civil surgeon and can perform your exam)

Completing Medical Forms

You are required to complete Part 1 of the Form I-693.

Do NOT sign the form until you are instructed to do so by the Civil Surgeon.

The designated civil surgeon will complete and sign Form I-693, Report of Medical Examination and Vaccination Record, and seal the document in an envelope. You must submit the sealed envelope to USCIS as directed in the Form I-693 instructions.

Note: Do not break the seal or open this envelope. The USCIS will not accept Form I-693 if the envelope has been opened.
